Partnering with People at Home

Supporting people in finding home settings that feel safe, comfortable, and truly their own.

Living with Support

Home Options Designed Around You

Each person has different routines, needs, and goals. 3Rivers offers several ways for people to live safely and comfortably at home, with support tailored to the individual.

Some live in a family-style home with a Shared Living Provider. Others live with peers, or independently with flexible assistance. Some receive specialized support in structured settings. Across every option, we center dignity, independence, and meaningful connection.

What We Do

We offer a wide range of services designed to support each person’s goals, abilities, and daily needs. Every service is guided by our core values of respect, empowerment, collaboration, and community.

Shared Living

Shared Living offers a warm, household environment where a person lives with a trained Shared Living Provider. This setting supports close relationships, daily routines, and personal preferences, offering stability and a strong sense of belonging.

Group Homes

Group Homes are small shared homes with daily support from trained staff. They focus on daily living skills, community involvement, independence, and consistency. It’s an option for people who enjoy living with peers while having steady support available.

Independent Living

Independent Living supports people who live in their own homes and want help with wellness, daily tasks, planning, or community connection. Support ranges from occasional check-ins to more regular guidance — always based on what the person wants and needs.

ICF / Treats Falls House

ICF / Treats Falls House offer structured home settings with round-the-clock specialized support. Trained staff assist with personal care, health needs, behavioral support, community participation, and long-term goals. It’s a strong option for individuals who benefit from consistent, higher-level care.

Finding the Right Fit

Understanding Your Options

Each home-based option meets different needs. Some people thrive in a family-style home, others prefer peer settings, and some feel most comfortable living independently with occasional support.

For individuals needing more consistent care, structured settings offer stability and safety.

Our team can help you explore each option and choose what feels right.
